If you have a birthday, anniversary, special achievement, graduation or other significant milestone that you would like recognized, please fill out the form on the next page by clicking on the “Click here to start” tab.

When filling out the Request Form please give specific details (age being celebrated or year of the anniversary, etc). These details can be included in the “Details” sections of the Request Form.

Furthermore, if you are celebrating a wedding anniversary for 25 years of marriage and higher you may receive a certificate from the Prime Minister. At 50 years you may receive one from the Governor General of Canada and at 60 years of marriage you may receive a certificate from Her Majesty Queen Elizabeth II.

For birthdays, at age 65 you may receive a certificate from the Prime Minister. At 90 years of age you may receive one from the Governor General and at 100 years of age you may receive a certificate from Her Majesty Queen Elizabeth II.
